Have the following information ready:

• Your wireless number
• ZIP code associated with your wireless account
• Device make, model and IMEI
• Cause and date of loss, breakdown or damage

Service fee payment method, if applicable (by credit card, debit card)

In certain cases, additional documents, such as a government-issued identification may be required to process your claim.

You may be required to disable the Find My/Find My iPhone or any other security features prior to making a claim and before sending back a claimed device.

Your 15-digit IMEI number can be found in your device’s original packaging or purchase receipt. If your device is still working, dial *#06# to display the IMEI number. For some manufacturers it’s found in the settings. You can also call your mobile carrier to get it.

We encourage you to file a claim as quickly as possible to minimize your downtime. Refer to your coverage documents for details on the specified time lime to file a claim.

Yes, please read the information below about your plan’s claim limits:

• Unlimited mechanical breakdown claims after the manufacturer’s warranty expires

• Up to two front screen repairs or back glass repairs per 12 rolling months at an Assurant-authorized repair center*

• Up to two device replacements** per rolling 12 months for accidental damage claims

*Limited to certain smartphone and tablet models. See fastclaim.com/totalwireless for additional details. Applies when and where repair service is available. Otherwise, a replacement is available under Accidental Damage (All Other) for a $29-$349 service fee depending on your device tier.

** Device replacement will be a new, refurbished, or recertified product of like kind and quality.

Once your claim is approved and your service fee has been paid, replacement devices will be shipped via next-business-day delivery, when available, at no additional cost. 

All device repairs and replacements will be with reconditioned parts and devices of like kind and quality. If reconditioned parts or devices aren’t available, then repairs and replacements will be with new parts and devices of like kind and quality.  Device color may vary depending on availability.

Once your claim has been approved, the claimed device becomes property of the company providing your coverage and must be returned to Assurant using the instructions provided. If you don’t return your damaged device to Assurant within 10 days from the date you receive your replacement, you’ll be charged a fee equal to the retail price of the replacement device plus applicable shipping costs. Please refer to your coverage documents for details.

You will find what you need to return your damaged device in the replacement device package.

If you’re returning an Apple® device please refer to the Apple Device Owner's card, enclosed with your replacement device, for instructions on how to unlock your device, disconnect it from iCloud, and erase its content before returning it to us.

You can also track the claimed device to make sure we received it; just use the Track My Claim function in the Home page.

If you misplaced the prepaid mailing label needed to return your damaged device, you can print a replacement mailing label following these three easy steps:

1. From the Homepage, start at the File, Continue or Track My Claim box. Enter your mobile number or serial number, complete the security check and click Submit. Once your number has been verified, click Track My Claim.
2. Enter your ZIP code and click Submit.

Click the My Return Instructions tab located at the top. Then click Return Receipt Label to download your label in PDF format
